Segmental Lumbar Extension T10-S1 (prone)

Position of the therapist:

  • Standing at the long edge of the bench on the side to be examined/treated

Position of the patient:

  • Prone lying position

Hand placement:

  • Fixating hand on L4 with a pinch grip in ventrocaudal direction
  • Contralateral leg grabbed with the other hand above the knee joint from the inside, moving the hip into its maximally closed packed position of internal rotation, adduction and extension

Movement:

  • Homonymous 3D movement with equal components of extension, ipsilateral lateral flexion and ipsilateral rotation by bringing the hip further into extension and adduction until the fixating hand wants to move
  • In order to test each component of intervertebral motion, slightly move out of the end-range and now test extension between L4 and L5 by pushing the spinous process of L4 ventrocaudally
  • To assess the rotation component, move to the contralateral arch of L4 and push it ventrally
  • For the lateral flexion component, put the thumb between the spinous processes of L4 and L5 and perform lateral flexion by moving the hip further into adduction
