Lumbar 2D Gap Manipulation (protection)

Position of the therapist:

  • Standing at the long edge of the bench
  • Bench height slightly higher than knee level

Position of the patient:

  • Side-lying position close to the edge of the bench facing the examiner
  • Bottom leg completely extended, the other leg bent with its foot hooked behind the other knee
  • Lumbar spine brought into
    • slight extension to protect the IVD in case of herniation
    • slight flexion in case of spondylolisthesis
  • Head rotated with trunk
  • Arms crossed in front of the chest

Hand placement:

  • Therapist sticks one hand through the patient’s crossed arms with the forearm placed against the patient’s coracoid process
  • Fixating hand on lower vertebra of level to be manipulated
  • Other hand rotates trunk from above until resistance is felt
  • Fixating hand creates pre-tension by rotating pelvis in opposite direction
  • Hypothenar eminence of fixating hand placed on transverse process of lower level

Impuls:

  • Patient is rolled towards the therapist so that the elbow of the fixating hand can almost entirely extend while making sure to maintain the pre-tension. Impuls using a contra-technique of the fixating hand
