Porcellini Test| | Posteroinferior Labrum Tear

Execution:

  1. The patient is standing or sitting and the examiner positions himself behind the affected shoulder
  2. The patient flexes his shoulder to 90°, 10-15° of adduction and internal rotation so that his thumb is pointing downwards
  3. The examiner stabilizes the scapula with one hand and applies isometric resistance against the patient’s arm moving into elevation
  4. The test is now repeated with the examiner applying an anterior pressure to the patient’s humeral head to prevent the humeral head from subluxating posterioly

 

Positive Outcome: Pain with or without weakness during the first part of the test is reduced or eliminated during the second part

Comment: Although this test has very promising accuracy, it has not been validated by other studies yet. It is unclear how the inter-rater reliability was determined in the study.
