Transdeltoid Palpation / Rent Test

Execution:

  1. The patient is in sitting position with the arm as relaxed as possible
  2. The examiner stands behind the patient and palpates anteriorly to the anterior edge of the acromion with one hand
  3. The other hand grasps the patient’s elbow and brings the patient’s arm into extension 
  4. The examiner carefully internally and externally rotates the arm to palpate the rotator cuff tendons

Positive Outcome: A tender depression/rent anterior to the acromion is felt

Comment: Another study by Lyon et al. (1992) had conflicting results and was excluded in systematic review on the basis of clinical grounds so despite the high likelihood ratios we regard the clinical value as moderate
