Hyperextension-External Rotation (HEER) Test

Execution:

  1. The patient lies in supine position with the legs hanging freely over the edge of the bench
  2. The patient is asked to flex the unaffected leg maximally and hold it in the flexed position
  3. The affected hip is hyperextended by the examiner while the hip is passively moved into external rotation

 

Positive Outcome: Reproduction of anterior hip pain is consistent with a positive test result for instability

Comment: The HEER Test seems to be a moderate test to confirm and exclude hip microinstability. However, further validation studies are needed to confirm these findings.
