The RPS form serves as a tool to structure the information you get during patient history taking.
- Body Functions: Describes actual anatomy and physiology/psychology of the human body.
- Activities: General activities that are being carried out in daily living and that are limited
- Participation: Activity that has a social meaning for the patient like hobby or job
- Personal Factors: Everything inside of a patient that cannot be taken away from him and that has an influence on recovery, e.g. coping style, stress level, anxiety, level of motivation etc.
- Environmental Factors: Factors that are not within the person’s control, such as family, work, government agencies, laws, and cultural beliefs
