Nakajima N-Test / Jolt Test

Execution:

Following is the description for assessing the right knee

  1. Patient is in supine lying position with the affected knee flexed to 90°
  2. Using the right hand, the examiner picks up the leg by grasping the lateral side of the ankle and applies internal rotation
  3. The left hand rests on the lateral aspect of the patient’s femur and applies a valgus force
  4. The thumb of that hand is placed on the posterior aspect of the head of the fibula
  5. Then the knee is gradually extended

 

Positive Outcome: The tibial plateau suddenly subluxates anteriorly and medially with a palpable jolt that usually occurs at around 40°-20° of flexion. The patient pay also report the feeling of “giving-way” as felt during activities
