Cluster of Sutlive

The Cluster of Sutlive is a 5-item test cluster that can aid in the preliminary diagnosis of hip osteoarthritis in patients who complain of unilateral hip pain. This clinical prediction rule aims to reduce the need for radiographic imaging and can provide clinicians with a tool to initiate early conservative management.

The different tests of the cluster are:

  • Hip Scour Test:
  1. Patient is in supine lying position
  2. The examiner flexes the hip to 90°, moves the hip towards the contralateral shoulder and applies an axial load through the femur with their own body
  3. Then, the hip is moved into internal rotation and compression
  4. At last, the hip is moved into adduction and compression

Positive Outcome: The patient’s familiar groin pain is reproduced

 

  • Passive internal rotation:
  1. Patient is in prone lying position with 90° of knee flexion
  2. The examiner holds an inclinometer proximal to the lateral malleolus
  3. The examiner brings the patient’s hip into maximal internal rotation (stop as soon as the contralateral buttock begins to rise from the table)

Positive Outcome: Hip internal rotation ≤25°

  • Hip Pain while squatting
  • Pain during active hip flexion
  • Pain during active hip extension

Comment: The Cluster of Sutlive is a helpful tool to diagnose hip OA in case of 3 or more positive tests. However, the confidence intervals were wide, which is why we assigned it a moderate clinical value
