Execution:
- The patient is standing or sitting and the examiner positions himself behind the affected shoulder
- The patient flexes his shoulder to 90°, 10-15° of adduction and internal rotation so that his thumb is pointing downwards
- The examiner stabilizes the scapula with one hand and applies isometric resistance against the patient’s arm moving into elevation
- The test is now repeated with the examiner applying an anterior pressure to the patient’s humeral head to prevent the humeral head from subluxating posterioly
Positive Outcome: Pain with or without weakness during the first part of the test is reduced or eliminated during the second part
