Posterolateral Rotatory Drawer Test | Posterolateral Rotary Instability

Execution

  1. Patient is in supine lying position with the arm to be tested overhead and the examiner stands at the head of the table
  2. The patient’s arm is fixated at the wrist between the examiner’s upper arm and body with the wrist in neutral rotation
  3. The patient’s humerus is fixated by the examiner’s outer hand with the thumb over the patient’s triceps tendon
  4. The examiner grasps the patient’s forearm with the index and middle finger of his other hand along the anterior aspect of the proximal radius and the thumb on the subcutaneous border of the ulna.
  5. When the patient is relaxed, the examiner applies a posterolaterally rotatory force to the proximal radius with the fingertips of the inner hand at various angles of elbow flexion.

Positive Outcome: Dislocation of radial head (dimple in the skin), which is maximal at around 40° of elbow flexion
