Malleolar Zone Algorithm | Ankle Fractures in Children

The Malleoloar Zone Algorithm was developed in order to rule out fractures after ankle injuries in children under the age of 18. This includes fractures of the tibia, fibula or talus.

The proposed algorithm for the tests goes as follows:

Positive Outcome:

  1. Tenderness on the fibula proximal to the malleolus
  2. Bony tenderness over any area of the lateral or medial malleolus and the child is not able to walk 4 steps
  3. Bony tenderness over any area of the lateral or medial malleolus and swelling over the lateral or medial malleolus
