Clarke’s sign / Patellar Grind Test

Execution:

  1. Patient is in supine-lying with the affected knee extended and the quadriceps relaxed
  2. The examiner pushes his web space of the thumb against the base of the patella and applies downward pressure on the patella
  3. The examiner then asks the patient to contract his quadriceps as if they were to extend the leg, while keeping the pressure on the patella

 

Positive Outcome: Reproduction of pain during the movement of patella or the inability to complete the test by the patient
